Keep the weight off with a long-term plan
Weight Loss Maintenance After GLP-1 Treatment
Reaching goal weight is a milestone, not the end of treatment. Appetite biology can return after GLP-1 medication is stopped, making a structured maintenance plan essential.
Why regain happens
Weight loss triggers biological adaptation: hunger may rise, satiety may weaken and energy expenditure may decline. When semaglutide or tirzepatide is stopped, the appetite-regulating benefit can disappear. Regain is not proof of weak character.

Components of maintenance
Medication plan
Continue, adjust, transition or stop only with clinician guidance. Some patients need long-term treatment.
Protein and strength
Maintain adequate protein and resistance activity to support muscle, function and energy expenditure.
Early-warning range
Define a small regain threshold that triggers action before a larger relapse.
Sleep treatment
Address OSA, insufficient sleep and fatigue that can undermine appetite control and activity.
When insurance stops coverage
Do not wait until the final dose. Discuss authorization renewal, appeal when appropriate, a covered alternative, a lower-cost oral option or a transparent self-pay plan. Abruptly improvising dose intervals or using unverified products can create risk.

Frequently asked questions
Must everyone remain on a GLP-1 forever?
No. Duration is individualized, but obesity is chronic and many patients regain after stopping.
Can I use a lower maintenance dose?
Sometimes, depending on product labeling, response and clinical judgment. Do not self-adjust.
What if hunger returns?
Contact the clinical team early to assess adherence, sleep, nutrition, dose, medication access and alternative causes.
