Dose escalation and tolerability
Zepbound starts at 2.5 mg weekly and is escalated gradually under its label. The starting dose is for initiation, not maintenance. Dose choice should reflect response and tolerability; the highest dose is not required for every patient.
Common adverse effects include nausea, diarrhea, constipation, vomiting, reflux and abdominal discomfort. Screening includes pancreatitis and gallbladder history, kidney and hydration risk, severe gastrointestinal disease, pregnancy, medication interactions and MTC/MEN 2. Tirzepatide has product-specific guidance for oral contraceptives during initiation and dose escalation.
