When phentermine/topiramate may fit
It may appeal to a patient who prefers a pill, has no relevant contraindication and needs a lower-cost option. It can reduce hunger, cravings and portions. The FDA-approved extended-release combination is not identical to separately prescribed immediate-release components.
Pregnancy prevention and testing are critical because topiramate exposure can harm a fetus. Higher-dose treatment must be tapered rather than stopped abruptly.
When a GLP-1 pathway may fit
Semaglutide or tirzepatide may receive priority when greater average loss, diabetes, cardiovascular risk or Zepbound's OSA indication is especially relevant. Gastrointestinal tolerability, long-term coverage and the ability to continue treatment must be considered.
